You will be referred to the oral hygienist when:

The gum disease periodontal disease has been established which has to be taken care of by an oral hygienist. Your gums will be inflamed and infected. The bacteria that cause this infection damage the fibers that hold the teeth into the jawbone. The oral hygienist will carefully establish the condition of your gums and discuss this with you. According to this examination a treatment plan will be established so you know how many appointments you will need and what the costs will be.
